Assessment Process
When the decision has been made that home care is necessary, Tender Hearts will take the time to make the assessment process comfortable and understandable for the client and family.
- A personal visit where the client presently resides (at home, assisted living community, nursing home)
- Making the client an active part in discussing a personal care plan that best meets their needs
- Discuss medical and pertinent job care requirements
- Helping to decide the number of hours and days to achieve the proper care
- Finding out about the client’s likes and dislikes
- Assessment of the home for safety
- Providing information and answering any questions
- Choosing the caregiver that best fits the needs and personality of the client
- Assuring the client that the caregiver and the plan of care can be changed at any time
